Why Revision Breast Surgery?

Both primary breast augmentation and breast reconstruction are well known for their reliably positive results, especially when safely performed by a qualified plastic surgeon. But no surgery comes with a guarantee. If you’re experiencing any of the conditions listed here, revisionary breast surgery may be the preferred remedy. Some women decide that removing their breast implants is the best choice and Dr. Sisco has the experience and specialized training to perform that procedure, too.

Along the way to dealing with breast cancer, I discovered a team of wonderful doctors who guided me to make the best decisions about my health first — but to also make me feel whole and happy in the end. Dr. Sisco especially had the patience to help me overcome some problems, and I felt more than pleased with the reconstruction, even in the early stages. I now look better than when I began this journey.

Capsular Contracture

Before & After Breast Lift (Mastopexy) Case 11 View #1 View in North Shore, IL
This patient, shown before revision breast augmentation with lift, presented with significant capsular contracture in her left breast.

Although still a relatively uncommon occurrence, capsular contracture is a well-known complication related to breast implants. Any time implants are added to the breasts, whether through breast augmentation or reconstruction, the body creates a “capsule” of tissue around the implants. This natural immune response is the body’s way of sequestering the implant, a foreign object, from the rest of the body’s systems and structures. However, occasionally the capsule can develop into a thick layer of scar tissue, causing breasts that are extremely firm to the touch. They may also look unnaturally round or especially far apart. Although capsular contracture isn’t a medical emergency, it can be physically uncomfortable and the appearance of the breasts can certainly be quite distressing.

Capsular contracture can be caused by infection or implant rupture, although often it doesn’t have a discernible cause.

Malposition

“Implant malposition” is a general term that refers to any case wherein the breast implants aren’t positioned correctly. Malposition may occur as the result of a surgical error or by implants that migrate, on their own, away from their original positions at any point after surgery. Malposition can appear in any of the following iterations:

Unsatisfactory Results

Occasionally, a patient may pursue revisionary surgery simply because her results just aren’t what she had in mind. Whether her implants are too small, too large, or don’t fit her lifestyle, a change of heart is always possible. Revisionary surgery is usually indicated when a patient is unhappy with her final outcome.

The Life of a Breast Implant

Although today’s implants look and function beautifully for much longer than previous implants, they don’t last forever. Both silicone and saline breast implants begin to show their age within about a decade of the primary surgery. Over time, their silicone shells can weaken, making them more susceptible to rupture or leakage and leading to complications such as capsular contracture. If they don’t leak, they may simply start to sag. Once the effects of aging become noticeable, revisionary surgery with an “upgraded” pair of implants or total removal of implants is appropriate.

The Surgical Experience

There are several factors that work together to make revisionary surgery more complex than primary augmentation or reconstruction. The presence of scar tissue, for example, can make secondary surgery tricky. The skin elasticity and quality and quantity of breast tissue are also sometimes compromised in women who have had breast implants for some time. It’s vital to choose a board-certified plastic surgeon who understands the nuances of breast revision and has the necessary experience to create the results you want.

Each revision surgery is unique. Due to its additional complexity, revisionary surgery is closely related to reconstruction. Dr. Sisco performs your revision while you are under general anesthesia, ensuring your complete comfort for the duration of the procedure. Different concerns require different techniques to be fully remedied. For example, if your implants have migrated below your breast crease, Dr. Sisco moves the implant back to its original position and may use a biocompatible material to support the implant and prevent the problem from reoccurring.

After surgery, you’ll need to take at least 1 week off from work and other daily activities. Dr. Sisco recommends waiting longer if your job is physically demanding. While you recuperate, the same rules as your primary surgery typically apply — sleep on your back as much as possible, avoid high-impact activities, and take your prescribed pain medication as needed. By choosing Dr. Sisco and caring for yourself properly after surgery, you can ensure your results meet — and hopefully exceed — your expectations.

I've had nothing but a positive experience with Dr. Sisco and his staff. I required four surgeries for a double mastectomy with reconstruction. Dr. Sisco held my hand through the entire process, as well as, handling some insurance issues I faced due to our chosen treatment plan. He's an exceptional surgeon and has a wonderful bedside manner. I cannot recommend him enough.

I had double mastectomy 3 years ago. The reconstructive surgery was completed after my chemotherapy was completed. Dr. Sisco did an outstanding job. I am so happy with the results. I look and feel good. His staff was supportive and took good care of me. The reconstruction of the nipple was painless and looks great. I would recommend Dr. Sisco to anyone.

I found Dr Sisco to be a very skilled surgeon in breast reconstruction. Mine was a bit more difficult as I had a reconstruction of the left breast after radical mastectomy, a nipple tattoo and a lift of the right breast to create semitry. I found Dr Sisco and his staff very professional and extremely kind!! He has very impressive credentials and I would highly recommend him ! Sincerely, Marlene Gaggiano

I had breast reduction surgery and am very thrilled with the results and with Dr. Sisco and his office. I was referred to Dr. Sisco by my primary care. I did take a minute to go online and see his profile on his website. The before and after photos impressed me. Knowing what the "after" would look like was important to me. Dr. Sisco is very easy to talk with and was patient with any questions or concerns I had. His office staff is very pleasant and were thorough making sure I was prepared for surgery. My decision to have surgery wasn't overnight, and I have no regrets. I am thankful to have a wonderful doctor, and his "team", who supported me through the process.

Dr. Sisco has been an integral part of my breast cancer journey and return to “normalcy.” Over the past year, from diagnosis through various reconstruction surgeries, he has been a stellar support, an absolute professional, and an innately talented surgeon. His work and bedside demeanor have been stellar, and he has left me looking (and feeling) far better than I could have expected. I was diagnosed with chondrosarcoma in my mandubrium in Jan. 2017. My thoracic surgeon referred me to Dr. Sisco for the reconstructive part of my 12 hour surgery. From my first consult to follow up, Dr. Sisco and his staff have been the most professional, compassionate and dedicated individuals I've had the pleasure of working with when it comes to healthcare. I've worked in healthcare, and I know it can be a thankless job and it can wear you down. Dr. Sisco and his staff were always positive, pleasant and so upbeat about my recovery. I spent 8 days in the hospital, and I can say at least twice a day, either Dr. Sisco or one of his staff would stop in my room to check in on me and my progress. Anyone who has been diagnosed with cancer or has fought the battle alongside a loved one, knows how devastating it is to hear that diagnosis for the first time. And then to have to go through consultation after consultation with a team of doctors is overwhelming, especially when you feel like you can't catch your breath. Dr. Sisco and staff made my diagnosis less scary. He explained step by step what he was going to do. He did not rush through the explanation. He gave me time to digest what was said and answered every question with patience and compassion. I also appreciate that Dr. Sisco has allowed me to decide when I would like to follow up with after care. I will need some minor surgery to fine tune the area in which bone, tissue and skin were replaced on my chest. He understood that I was not up for any surgery so soon after major surgery. I appreciate that I was not pressured to schedule that right away, and that I was given the choice of the timetable in which to return; when I felt comfortable. I can not thank Dr. Sisco and staff enough for the peace of mind they have given me throughout this diagnosis and treatment. I highly recommend Dr. Sisco and his staff! -A. Hauer
